This variation of the Item spell turns it's targets into patches sewn into the inside of a coat, cloak or robe. In essence, this spell is used to build a magic item known as a Robe of Useful Items (though it isn't always a robe).

When such a patch is torn free, the object or creature appears in the hand if it's small enough, or it appears in an adjacent hex of the user's choice.

Repeating Useful Item, Useful Creature and Useful Item are always sold together as a package deal.
